Member School Clinic Request Form

Please note, **NEW THIS SEASON**, participants must register online for the clinic, anyone needing education credit will also need to pay the ed credit fee of $45 online. An email with the link will be sent to you once the clinic is scheduled (pending clinician availability). Registration will stay open roughly 5 days before clinic date (depends on the day of the week), and group size is a maximum of 8.

No registrations will be accepted the day of.

The price for each clinic group is $450 and we will send an invoice after the clinic(s). If you are passing the cost of the clinic(s) onto your instructors you will need to collect the monies and send PSIA-NW one check or pay by credit card.

CANCELLATION POLICY: CLINICS MAY BE CANCELLED UP TO FOUR WEEKS PRIOR TO THE DATE OF THE CLINIC. IF THEY ARE CANCELLED AFTER THAT TIME THE FULL AMOUNT OF $450 PER CLINIC PER CLINICAN WILL BE DUE.
