Please use the form below to request an on-snow level I exam. Only accredited trainers can give the level I exam. If you are a school that does not have an accredited trainer to lead your level I exam please contact us. Level I Exam Request Form

The on-snow portion of the school supported Level I exam is $45 per participant. Once this exam is setup, the individuals go online and register (psia-nw.org) for the exam before the registration closes. This exam is only for your staff and the maximum size is 6. Please note, registration closes **TWO WEEKS** before exam date. If we have to re-open registration for any reason the cost will increase to $60.

  • Please enter the email for the person who is requesting the Level I exam.
  • Please list which snowsports school this Level I exam will be held at.

  • This information will be provided to participants who register so they know where to meet you.
  • For each discipline, you will need to fill out a new request form.
  • If you do not know yet please give us your best guess. Group sizes should have at least 3 and no more than 6 per accredited trainer.
  • Please provide a name and mailing address where we should send the Level I materials to. Allow two weeks for packet to reach you. Included in each packet is the roster for participants to sign, certificates, and pins.
