Level I Exam Request Form

Please use this form to request a Level 1 assessment. As a reminder, this is a two-day exam and once up on the calendar, any member is able to register for the exam, regardless if they work for your school. And generally, we will be able to have more than one group if that occurs. We will open up waitlists as we see the numbers increase. Encourage people to put themselves on waitlists, that is the only way we know we need to add another group. Once this exam is setup, the individuals go online and register (psia-nw.org) for the exam before the registration closes. Please note, registration closes **TWO WEEKS** before exam date so we can put the assessment packet together and mail it out to the assessor(s). Any questions, please contact us at info@psia-nw.org.

  • Please enter the email for the person who is requesting the Level I exam.
  • Please list which snowsports school this Level I exam will be held at.
  • Using the dropdown below, confirm the resort will (or will not, or you don't know) support lift tickets for anyone who does not have a pass at the resort.
  • Please note, registration closes **TWO WEEKS** before this date. (for additional dates please fill out another form)

  • This information will be included in the event confirmation for members registering for the assessment.
  • For each discipline, you will need to fill out a new request form.
  • Please provide a name and mailing address where we could send the Level I materials to. Sometimes it is better for us to send it directly to the resort than the examiner, depending on timing. We will communicate with you if the packet needs to be mailed to the resort, ensuring it can then be given to the examiner.
