Event Application Instructions: 

1.    The application is in PDF format, you will need Adobe Acrobat Reader to open the file.  For a free version of Adobe Acrobat Reader, click on the image below and follow the directions to download it.

2.    When you have Adobe installed on your computer, open the application form.

3.    Print the form, fill it out, sign it, have your Training Director sign it if applicable, and mail it or fax it to the PSIA-NW office.
