We accept faxed applications complete WITH liability release signature and credit card information as well as emailed applications WITH liability release signature and with or without any credit card information – if you prefer to not email your credit card payment information please phone PSIA-NW with the information. Of course you may also fill out your exam application and fax it, email it or place it in the mail.

If you fax or email your application for an  exam, you will receive an email back from us that we have successfully received your application. If you do not receive an email from us, please call PSIA-NW to confirm receipt. This is the best way to insure we have your information and you have been added to the exam roster for your chosen exam date.
